Depending on the version of Adobe Acrobat you have on your computer, you may be prompted with the message "This PDF requires a newer version of Acrobat. Copy this URL into your browser.” This newer version will allow you to submit the form electronically.

You will know that the form was submitted when a menu box appears titled “Select Email Client.” Select your option, click ok and your form will be submitted. I will send out email confirmations every Friday to everyone that submitted a form that week to confirm that I have received your form.
